The difference in this case is that in KDE you will find two programs: KView and Kuickshow. Then, if their features do not fulfill your needs, you can install a third-party image viewer: PixiePlus, Showimg, Kimdaba, Gwenview or maybe GQView or GThumb. I would say that the "only" problem are file associations: right now I think no image viewer associates itself with images once it's installed and it's quite painfull to manually associate it with all image types.
